Human Services & Veteran Affairs

The Sullivan County Veteran Affairs office provides assistance to veterans and their families:

• In applying for state and federal veteran’s programs and benefits.

• Is the point of contact for the veterans and regional veterans administration office.

• Distributes flags and markers for county veterans graves.

• Assists in accessing discharge records, awards, personnel files from service.

• The director must maintain accreditation and continuing education as a Veteran Service Officer (VSO).

The Sullivan County Human Services office coordinates many different programs with other outside agencies which provide direct services to County residents. Through the Human Services Development Fund (HSDF), State funded with an annual allocation, this office contracts with the following agencies to provide the services listed:

• Area Agency on Aging - Home delivered mails, personal care, and home support.

• TREHAB - Local housing assistance caseworker support.

• Sullivan County Victim Services - Victim transportation.

• Sullivan County Dental Clinic - Sliding scale fee payment program support for the under and uninsured.

• The Children's House - Protective services forensic interviews and medical exams.

• State Food Purchase Program (SFPP) – Provides State funded allocation dollars to Sullivan County Food Pantry and Trehab Food Pantry to purchase food for distribution to families in need.

• The Emergency Food Assistance Program (TEAFP) – Federal Surplus Food is made available for distribution to families in need through Trehab Food Pantry in conjunction with SFPP.

Housing Assistance

The Emergency Food and Shelter Program, a Federal funded assistance program, can provide a one (1) month mortgage or a one (1) month metered utility payment or vendor minimum delivery amount of a non-metered utility (Wood, Oil or Propane if the primary heat source). Sullivan County Households can be assisted once a year through this program while funds are available and on a first come first serve basis.

Assistance Programs

Sullivan County Assistance Program is funded through PHARE and PA Housing Finance Agency. This program may provide up to 3 months rent. Applicants must meet income limits and have a formal lease.

Services Provided Through Bradford County Human Services

• The Administrative Entity (AE) is responsible for:

• Eligibility determinations for intellectual disabilities and autism

• Monitoring of waiver services

• Oversight of Intellectual Disability service delivery in Bradford and Sullivan Counties

• Monitoring of provider compliance with State and Federal regulations

• Managing State and County funding allocations to maximize services within the allocation

Intellectual Disabilities Department

The Intellectual Disabilities Department is divided into two sub-divisions; the Administrative Entity and the Supports Coordination Organization. The program provides support services to persons with intellectual disabilities and autism and their families to support them in leading full lives in their own communities. The department is responsible for state mandated programs as administered by the state Department of Human Services and the Office of Developmental Programs.

Early Intervention Services

Formerly known as the “Birth to Three Program,” the Early Intervention Services group:

• Determines eligibility

• Coordinates supports

• Develops services

• Provides funding for young children under the auspices of the Office of Child

• Development and Early Learning

The Department assures compliance with all State and Federal policies and guidelines.

Supports Coordination Organization

Bradford/Sullivan County Supports Coordination Organization (SCO) is responsible for:

• The development of the Individualized Support Plan and insuring the appropriate

• Implementation as required by the Pennsylvania Office of Developmental Programs

• Monitoring of provider and family compliance with the plan

• Monitoring consumer progress and safety

Mental Health Services

The mission of the County Offices of Mental Health is to ensure that every adult and elder with a serious mental illness and/or addictive disease, and every child and adolescent who abuses substances, and/or has a serious emotional disturbance will have the opportunity for recovery, growth, and inclusion in their community, have access to services and supports of their choice, and enjoy a quality of life that includes family and friends. This agency provides services and planning for the provision of services for individuals needing mental health supports and services.

Bradford/Sullivan County Mental Health Services include:

• Referrals

• Children and adolescent service

• System program (CASSP)

• Community hospital integration projects program

• Partial hospitalization

• Inpatient treatment

• Peer support

Drug & Alcohol Services

The Bradford/Sullivan Drug & Alcohol Single County Authority (SCA) is responsible for the delivery of Drug & Alcohol services in the area of prevention, intervention and treatment within Bradford and Sullivan counties. The SCA assesses the needs within the county and develops programs that are designed to meet those needs using the most efficient means possible. Some of those needs are best met by contracting for services with out of county providers. The SCA acts on behalf of the Bradford County Commissioners to manage State and Federal grants.

Services Provided

Drug and Alcohol services are important to those in need. The drug and alcohol program of Bradford and Sullivan Counties can assist those in need in the following ways:

• Evaluation and Referral

• Outpatient

• D&A Partial Hospitalization

• Inpatient Rehabilitation

• Halfway House

• Intensive Case Management

• Methadone Maintenance

In addition we offer special and immediate services for:

• Pregnant Injecting Drug Users

• Pregnant Substance Abuse Users

• Intravenous Drug Users

• Addicted Women with Children
